COVID-19 SOS Directive Form and Wallet Card

 

If you decide that you want a COVID-19 SOS Directive, the next step is to fill out the form listing your wishes. You may use a lawyer if you want to but having a lawyer’s help is not required.

These are the steps to complete your COVID-19 SOS Directive

  2. Feel sure that you understand all of the information and that you want to have a COVID-19 SOS Directive.
